About Okayform
Okayform gives you free printable waiver, consent and release form templates. You can download each template as a PDF or a Word file, then print it and have people sign it on paper.
What you can use today
The template library is free to use. It has waiver and release templates and consent form templates for US businesses.
You don't need an account to download a template.
What is coming
Online signing and stored signed records are not available yet. If you want them, you can join the early-access list from any template page. We'll email you when they're ready.

Vinay Koshy runs Okayform. He is the founder, and he builds tools for signed forms.

Okayform is not a law firm, and the templates are not legal advice. No lawyer has reviewed them yet. Talk to a lawyer in your state before you rely on a form.
